P228DFuel Pressure Regulator A Exceeded Control Limits - Pressure Too High

P228D means the powertrain control module (PCM) commanded the fuel pressure regulator for circuit A to lower pressure, but the actual fuel pressure stayed above the commanded value beyond the allowed control range. The PCM detected that its pressure command and the measured pressure did not agree, so it flagged the regulator as exceeding its control authority on the high side.
